29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Throughout job site - On or about November 19, 2022, the employer supervisor instructed his employees to clean up the roofing debris below the roofers on the roof 20 feet high without the use of head protection exposing them to truck-by hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Top of roof West side - On or about November 19, 2022, the employer did not ensure the employees on the roof of a two-story apartment building were protected from falling with the use of a fall protection system exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Front of building - On or about November 19, 2022, the employer instructed his employees to use the extension ladder to carry shingles to the top of the roof when the ladder side rails did not extend 3 feet past the roofs edge exposing them to fall hazards.
