29 CFR  1904.4(a): The employer did not record each work-related fatality, injury, or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:   On or about October 6, 2022, an employee was hospitalized after dropping a steel lid on his toe.  The employee received medical care for the injury and missed 2 weeks of work.  This injury was not documented on an OSHA 300 or equivalent form.

29 CFR  1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training, and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup, or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Employees engaged in maintenance activities at Masterhand Milling at 1502 E Walnut St, Lexington, Nebraska were exposed to electrical shock, crushing, and amputation hazards in that the employer did not establish a program that incorporated energy control procedures, employee training, and periodic inspectors to prevent employee exposure to unexpected energization or startup, or the release of stored energy while performing servicing or maintenance.  Instances of employee exposure include but are not limited to:  A.)   Johnson & Sons JBH 9400 Bagger, on or about August 21, 2022.

Instances include but are not limited to:  A.)   Johnson & Sons JBH 9400 Bagger, on or about August 21, 2022.

29 CFR  1910.1200(e)(1):  The employer did not develop, implement, and maintain a written hazard communication program at the workplace which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  Employees who handled chemicals at the Masterhand Milling Facility located at 1502 Walnut St, Lexington, Nebraska, were exposed to chemical hazards in that the employer had not developed, implemented, and maintained a written hazard communication program at the facility.   The employer is exposing employees to various chemicals such as grease, gear oil, and WD-40, but has not created a written hazard communication program.

29 CFR  1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Employees who handled chemicals at the Masterhand Milling Facility located at 1502 Walnut St, Lexington, Nebraska, were exposed to hazards in that the employer had not provided adequate information for the on-site specific chemicals, hazards associated with those chemicals, hazardous chemical labeling, and measures that employees can take to protect themselves from these hazards. Chemicals used in the facility included but not limited to gear oil, bearing grease, and WD-40. These chemicals can cause eye, skin and respiratory irritation if contacted or inhaled.
