29 CFR 1910.212(a)(2):  The employer did not ensure general requirements for machine guards were in place:     (a)CCL Label/Sioux Falls, Inc. at 1209 West Bailey Street, Sioux Falls, SD 57104:  On or about and at times prior to November 21, 2022, the employer did not ensure that machine guarding was in place on the Clausing Colchester 15 Manual Lathe in the maintenance shop. This condition exposes employees to caught-in and amputation hazards during operation.   (b)CCL Label/Sioux Falls, Inc. at 1209 West Bailey Street, Sioux Falls, SD 57104:  On or about and at times prior to November 21, 2022, the employer did not ensure that machine guarding was in place on the the Dayton 6 x 9" Belt & Disc Sander in the maintenance shop. This condition exposes employees to caught-in and amputation hazards during operation.   (c)CCL Label/Sioux Falls, Inc. at 1209 West Bailey Street, Sioux Falls, SD 57104:  On or about and at times prior to November 21, 2022, the employer did not ensure that machine guarding was in place on the Slitter 716 Titan Slitter machine on the main production floor. This condition exposes employees to caught-in and amputation hazards during operation.

This condition exposed employees to shock hazards.
