29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about November 25 2022, at the jobsite located at 355 West Wisconsin Ave, Oconomowoc WI, employees were performing roofing work on a single story commercial building with pneumatic nail guns without safety glasses.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about November 25 2022, at the jobsite located at  355 West Wisconsin Ave, Oconomowoc WI, an employee was performing roofing work on a single story commercial building with a pitch of 7:12 with a fall height of 8.2 feet without any fall protection in place, thereby exposing the employee to a fall hazard.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about November 25 2022, at the jobsite located at  355 West Wisconsin Ave, Oconomowoc WI, employees were performing roofing work on a single story commercial building with a pitch of 7:12 with a fall height of approximately 8.2 without any fall protection in place, thereby exposing the employees to a fall hazard. Employees did not have fall protection training.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  On or about November 25 2022, at the jobsite located at 355 West Wisconsin Ave, Oconomowoc WI, employees accessing the roof to conduct roofing work from a ladder which did not extend at least three feet above the roof.
