Defective rigging equipment shall be removed from service.  37 Kalliste Hill, Great Barrington, MA 01002: On October 10, 2022, and at times prior, the employer had not inspected the rigging equipment for defects such as holes, tears or cuts and removed that equipment from use.

29 CFR  1926.1425(c)(1): The materials being hoisted must be rigged to prevent unintentional displacement.   37 Kalliste Hill, Great Barrington, MA 01002: On October 10, 2022, and at times prior,  materials being hoisted were rigged by slings that wrapped around the load (or "cradled" ) and not choked,  which enabled  unintentional displacement of the load to during lifting.
