29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On November 18, 2022, employees engaged in decking work without fall protection were exposed to unprotected fall hazards of approximately 40 feet to the ground below.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.   On November 18, 2022, employees using an extension cords to energize skill saws that had the outer sheathing pulled away exposing internal wiring were exposed to electrical shock hazard.
