29 CFR 1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. a) On or about November 30, 2022, in the machine shop, where employees are expected to fight incipient level fires, a fire extinguisher was blocked by brooms and metal rods and was inaccessible to employees.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about November 30, 2022, in the fabrication shop employees were performing machining operations using two lathes that did not have chuck guards: a) Lathe#1: Kingston Model: HD2290 SN: CH17888 used for heavy duty precision cutting to shape metal. b) Lathe #2: ACRA Model: 22160 SN: CH13062 used for heavy duty precision cutting to shape metal.
