29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about November 30, 2022, workers of Eric Cocical dba. Eric Cocical were repairing a skylight at a residential building located at 6433 Woodbine Court in St. Louis, Missouri.  They were exposed to fall hazards as they worked approximately sixteen feet above the ground.  They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about November 30, 2022, workers of Eric Cocical dba. Eric Cocical were repairing a skylight at a residential building located at 6433 Woodbine Court in St. Louis, Missouri. They were exposed to fall hazards while using a ladder to access the roof.  The ladder was not extended at least three feet beyond the landing or edge of the roof.
