29 CFR  1926.651(g)(1)(ii):  Adequate precautions were not taken to prevent employee exposure to atmospheres containing less than 19.5 percent oxygen and other hazardous atmospheres:   a)  On or about November 30, 2022;  where employees worked in an excavation with an existing sewer line present and precautions were not made to ensure hazardous conditions did not exist.

29 CFR  1926.651(j)(2):  Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided:  a)  On or about November November 30, 2022, employees were exposed to struck-by hazards while working in an excavation where debris was not kept at least 2-feet back from the edge.
