29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  On or about November 30, 2022, and at times prior thereto, employees were exposed to a struck-by hazard while utilizing a high-pressure air lance to search for a water line under the public roadway without eye and face protection.
