29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On December 2, 2022, at the above addressed jobsite, an employee was exposed to a struck-by hazard from falling roofing materials, tools, and debris when working below employees and material.  The employee was not wearing a hard hat.  No abatement certification or documentation is required for this item.

29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) On December 2, 2022, at the above addressed jobsite, an employee did not attach lanyard to the boom or basket of the Genie S-45 Telescopic Boom aerial lift when working approximately 25 feet from the ground.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) On December 2, 2022, at the above addressed jobsite, the employer did not provide training to each employee in order to recognize hazards associated with working from aerial and scissor lifts and to understand procedures to control or minimize those hazards.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
