On or about December 5, 2022, employees engaged in decking activities without fall protection were expose to unprotected fall hazards of approximately 10-feet to the ground below.

On or about December 5, 2022, the employer did not provide an extension ladder where employees could access and egress the residential building roof level, exposing them to potential fall hazards.
