29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) Worksite: On or about 12/6/2022,  an employee was exposed to fall hazards while standing on the mid-rail of a Skyjack SJ6832 RT 4x4, approximately 16 feet 7 inches above the lower level, without any means of fall protection.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) Worksite: On or about 12/6/2022, an employee was exposed to fall hazards while working from approximately 16 feet above the ground floor without fall protection.
