29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   On or about December 2, 2022 and times prior there to, at a commercial facility located at 20235 Rhodes Road in Spring TX, the employer failed to ensure that a powered industrial truck had manufacturer certification for the use of an attached man lift cage.
