29 CFR  1926.502(f)(2):Warning line systems did not consist of ropes, wires, or chains, and supporting stanchions erected in accordance with (f)(2)(i) through (f)(2)(v) of this section:  On or about December 8, 2022, employees were exposed to fall hazards in excess of six (6) feet where the warning line system on the the roof of Building B did not consist of ropes, wires, chains and supporting stanchions. The warning line was a line drawn on the roof.  Abatement Note:  Abatement certification is required for this item.  (See enclosed "Certification of Corrective Action Worksheet").

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) On or about December 8, 2022, employees were exposed to fall hazards in excess of six (6) feet while working on the roof section of Building B with no fall protection in place.  Exterior Solutions Group was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 Duty to have fall protection, which was contained in OSHA inspection number 1554879, citation number 1, item number 1 and was affirmed as a final order on February 14, 2022, with respect to a workplace located at 2301 S 16th St., Chickasha, OK 73018.
