29 CFR  1910.22(a)(1):  The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  On or about December 9, 2022 , in the Builder Shop area, where employees were performing activities related to carpentry:  (a)  extension cords were coiled on the floor exposing employees to trip and fall hazards;  (b) wood dust was allowed to accumulate on the floors exposing employees to slip and fall hazards.  (c) wood dust was allowed to accumulate on the floors, walls, electrical equipment, and other surfaces exposing employees to fire and explosion hazards.

29 CFR  1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present:   On or about December 9, 2022, in the Builder Shop,  employees using electrical equipment not designed for installation in a Class II, Division 1 area plugged into receptacles such as, but not limited to:  (a) Box G-12 with two receptacles on a fixed installation on top of a portable fire extinguisher, was providing power to a phone charger.     (b) flexible cord providing power to metal outlet box G-3 with two receptacles, mounted to the wall adjacent to "Do Not Forklift" sign , was providing power to extension cords.  (c) flexible cord providing power to metal outlet  box above box G-11, next to column, with two receptacles, was providing power to extension cords and sanders.    (d) extension cords were providing power to chargers plugs at outlet box G-11, next to  with two receptacles.   (e) flexible cord providing power to metal outlet box B1-5, with two receptacles , next to column .  (f) GFI receptacle mounted to column adjacent to boxes G-11, B1-5, and the one on top of G-11.  (g) flexible cord providing power to metal outlet box G-1 with two receptacles, mounted to the wall adjacent to "Do Not Forklift" sign , was providing power to extension cords.

Instances include, but are not limited to the following:  a) outlet box B1-5 providing power to equipment such as, but not limited to battery chargers  b)outlet box G-11 providing power to equipment such as, but not limited to cell phone charges and sanders  c) outlet box B1-1 providing power to equipment such as, but not limited to a fan  d) outlet box B1-11, fan and sanders plug to these boxes  e) outlet box providing power to dust collector located near Grizzly Industrial G0941 table saw  f) outlet box G-3 providing power to equipment such as, but not limited to a Bosh Sander by "Do not Forklift" sign  g) two metal outlet boxes with prepunched knockouts adjacent to Tank Room Pump disconnect used as pendants to provide power to sanders and extension cords

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   On or about December 9, 2022 , in the Builder Shop, employees were using flexible cords to supply power to equipment including, but not limited to the following:  a) an extension cord was providing power to the Table saw Grizzly Industrial G0941, Ser. 2021252.  b) an extension cord was providing power to a Bosh Sander plugged into to G-3 box by "Do not Forklift" sign.  c) extension cords was providing power to a fan and Bosh sanders by B-1, B-11 boxes, near paint cabinet and fire extinguisher.  d)  extension cords were providing power to chargers plugs at G-11 and the box on top of box G-11.  e)  flexible cord receiving power from a junction box mounted to the ceiling joists provided power to a DeWalt miter saw  f) extension cord providing power to battery chargers on lower shelf of cart  g) flexible cord providing power to metal outlet box G-1 mounted to the wall adjacent to "Do Not Forklift" sign  h) flexible cord providing power to metal outlet box G-3 mounted to the wall adjacent to "Do Not Forklift" sign   i) flexible cord was providing power to  pendant box where a dust collector was plugged  by  table saw Grizzly Industrial G0941.  j) flexible cord providing power to metal outlet box B1-5 , next to column .
