29 CFR  1910.178(l)(1)(ii):The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  a) Shipping Department: On or about December 12, 2022, and at times prior, the employer exposed employees to struck-by hazards, in that employees were not provided powered industrial truck operator training prior to being required to operate a "Toyota Material Handling Systems" HELI model forklift  to unload and separate unassembled golf carts.

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Assembly Area: On or about December 12, 2022, and at times prior, the employer exposed employees to chemical hazards, in that  a written hazard communication program was not developed and implemented for chemicals including, but not limited to, "Lucas" Synthetic SAE 75W-90 Gear Oil and "Sinopec" Marine Grease used by employees in the performance of assigned job duties.

29 CFR  1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Assembly Area: On or about December 12, 2022, and at times prior, the employer exposed employees to chemical hazards, in that hazard communication program training was not provided to employees who use chemicals including, but not limited to, "Lucas" Synthetic SAE 75W-90 Gear Oil and "Sinopec" Marine Grease in the performance of assigned job duties.
