On or about November 30, 2022,  an employee was exposed to falling 15-feet into the excavation when the employee was hoisted in the bucket of a Caterpillar Excavator to perform work on a pipe near grade level.

C) On or about December 5, 2022, an employee was exposed to cave-ins while working in an approximately 16-foot-deep excavation without protection.   D) On or about January 10, 2023, an employee was exposed to cave-in hazards while working in an approximately 10-foot-deep excavation without protection.    E) On or about January 20, 2023, two employees were exposed to cave-in hazards along the south wall of a 12-foot-deep excavation without protection.
