29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder  (a): Facility - A portable extension ladder was not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder. The horizontal distance from the top support to the foot of the ladder was approximately one-half the working length of the ladder. An employee fell approximately 12 feet when the ladder slid out from underneath, on or about December 12, 2022.  There is no abatement certification or documentation required for this item.
