29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about December 14, 2022, at a work-site located next to 3958 SR 43, Kent Ohio 44240; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 11 feet.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about December 14, 2022, during the installation of framing material at the job site located at 3958 SR 43, Kent Ohio 44240; the employer did not ensure employees working on the roof of a commercial location were protected from possible danger of eye injury from impact, or from flying objects from the employees cutting and installing framing material.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about December 14, 2022, during the installation of roofing material at a job site located at 3958 SR 43, Kent Ohio 44240; the employer did not ensure employees working on a roof of a commercial facility were protected from a fall of approximately 11 feet by utilizing a fall protection system.
