On the roof of a temporary residential worksite, at 1642 Fairfax Street, Jacksonville, Florida 32209: On or about December 7, 2022, the employer exposed two employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns.

On a 3:12 pitched roof of a temporary residential worksite, at 1642 Fairfax Street, in Jacksonville, Florida 32209: On or about December 7, 2022, the employer exposed two employees to fall hazards ranging from 9 to 15 feet, in that fall protection was not utilized by employees while sheathing and nailing down 2 x 4's.
