29 CFR 1910.147(c)(4)(ii): The employer did not develop lockout procedures that clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. On or about December 15, 2022 and at times thereafter, employees were exposed to electrical, hydraulic, pneumatic and other hazardous energy sources during maintenance and servicing activities and the employer did not develop lockout procedures that detailed the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls was acquired by employees. The training shall include: (A) Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. (B) Each affected employee shall be instructed in the purpose and use of the energy control procedure. (C) All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out. NOTE: The employer shall certify that employee training has been accomplished and contain each employee's name and date of training. - On or about December 15, 2022 and at times thereafter, employees were exposed to electrical, hydraulic, pneumatic and other hazardous energy sources during maintenance and servicing activities and the employer did not ensure each authorized employee received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control; each affected employee was not instructed in the purpose and use of the energy control procedure; and all other employees whose work operations were in an area where energy control procedures were used were not instructed about the procedure and the prohibition of reenergizing equipment which is locked or tagged out.

On or about December 15, 2022 and at times thereafter, employees were exposed to rotating parts, flying chips and/or sparks while operating the G & L Boaring Mill and the employer did not ensure a Lexan guard was installed. b. On or about December 15, 2022 and at times thereafter, employees were exposed to rotating parts, flying chips and/or sparks while operating the Lucas Boaring Mill and the employer did not ensure a spindle guard was installed. c. On or about December 15, 2022 and at times thereafter, employees were exposed to amputation hazards while operating the Hem-Saw and the employer did not ensure a sleeve or cover was installed over the unused portion of the blade.
