29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501    a.) On or about December 16, 2022, at 250 Cutter Court, Sarasota, FL 34278, employees in the process of removing blue tarp and re-roofing and were exposed to fall hazards of 9 feet 10 inches, in that, fall protection equipment was not installed or utilized.   All Weather Enterprises, Inc., dba All Weather Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10), which was contained in OSHA inspection number 1448741, citation number 1, item number 1, and was affirmed as a final order on 05-06-2020, with respect to a workplace located at Countryside Village Mobile Home Park- 9507 Red Run Drive, Tampa FL, 33637.

On or about December 16, 2022, at 250 Cutter Court, Sarasota, FL 34278, employees in the process of removing blue tarp and re-roofing, were exposed to fall hazards of 9 feet 10 inches, in that, there was a missing pan/hole on the roof which was covered but was not labeled to prevent employees from stepping into it.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a.) On or about December 16, 2022, at 250 Cutter Court, Sarasota, FL 34278, employees in the process of removing blue tarp and re-roofing and were exposed to fall hazards of 9 feet 10 inches, in that, the ladder used to gain access to the roof was not secured to the building structure.
