29 CFR  1926.651(b)(1): The estimated location of utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, shall be determined prior to opening an excavation:  a) 220 Buckton Drive, Lititz PA - On or about December 19, 2022, an excavator operator struck a 3/4-inch water line causing a water leak and flooding at the job site. The employer had not determined the estimated location of utility installations and/or any other underground installations prior to opening the excavation as required.   b) 220 Buckton Drive, Lititz PA - On or about December 19, 2022, an excavator operator struck a 2-inch natural gas line causing a gas leak at the job site. The employer had not determined the estimated location of utility installations and/or any other underground installations prior to opening the excavation as required.  No abatement certification or documentation required.

29 CFR  1926.651(b)(2): Utility companies or owners were not contacted within established or customary local response times, advised of the proposed work, and asked to establish the location of the utility underground installations prior to the start of an excavation:  a) 220 Buckton Drive, Lititz PA - On or about December 19, 2022, an excavator operator struck a 3/4-inch water line causing a water leak and flooding at the job site. The employer did not contact Pennsylvania One Call (811) and advise of the proposed work prior to the start of an excavation as required.   b) 220 Buckton Drive, Lititz PA - On or about December 19, 2022, an excavator operator struck a 2-inch natural gas line causing a gas leak at the job site. The employer did not contact Pennsylvania One Call (811) and advise of the proposed work prior to the start of an excavation as required.  No abatement certification or documentation required.
