Located at 8094 Chesapeake Place NW, Pickerington, Ohio:  On or about December 20, 2022, the employer did not ensure the scaffold footing was capable of supporting the loaded scaffold without settling or displacement when employees performed masonry work. Employees were thereby exposed to fall hazards of approximately 15 feet, 6 inches, in the event of a scaffold collapse.

29 CFR  1926.451(e)(1):  When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  a.	Located at 8094 Chesapeake Place NW, Pickerington, Ohio:  On or about December 20, 2022, the employer did not provide an access ladder for employees to use when they worked from the fabricated frame scaffold system to conduct masonry work.

29 CFR  1926.451(g)(1):  Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.  a.	Located at 8094 Chesapeake Place NW, Pickerington, Ohio:  On or about December 20, 2022, the employer failed to ensure that employees who performed masonry work from the second buck level of the fabricated frame scaffold system were protected from falling approximately 15 feet 6 inches to the ground level.

29 CFR  1926.454(a):  The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  a.	Located at 8094 Chesapeake Place NW, Pickerington, Ohio:  On or about December 20, 2022, the employer did not provide training for employees working from a fabricated frame scaffold system such as but not limited to; identifying fall hazards and falling object hazards, procedures for erecting and maintaining fall protection systems, procedures for erecting and maintaining the scaffolding and proper use of the scaffold.
