29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) EPI Breads located at 2650 Button Gwinnett Drive, Doraville, GA 30340, Throughout plant: On or about December 20, 2022, and at times prior to, the employer did not establish an energy control program for employees who performed service and or maintenance of equipment including, but not limited to, the Pillow Pack, Dump Table, Slicer and Outfeed Conveyor Belt.

29 CFR  1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points and rotating parts:  (a) EPI Breads located at 2650 Button Gwinnett Drive, Doraville, GA 30340, Outfeed conveyor belt in packaging area: On or about December 20, 2022, and at times prior to, employees were exposed to a caught between hazard, in that employees were not protected from the unguarded conveyor belt.
