One exit door in the front of the warehouse was blocked by a large fan and a pallet with a metal container full of concrete debris.

In the facility at 1 Industrial Park Road West: On and before December 21, 2022, two employees working in the warehouse and in the yard operated a Caterpillar 8386 and a Crown sit down forklift and the employees had not received practical performance training as required by the standard.

29 CFR  1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   In the facility located at 1 Industrial Park Road West in Oxford, MA: On and before February 10, 2023, employees were allowed to use 3M 8210V N95 respirators voluntarily for potential exposure to nuisance dust and were not provided with the information contained in Appendix D to 20 CFR 1910.134.
