29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about 21 December 2022, employees of Foreman Fabricators Inc. located at 4605 McRee Avenue in Saint Louis, Missouri were exposed to hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Several machines to include but not limited to a press brake, Bridgeport Milling Machine, and Clausing Horizontal Lathe were inadequately guarded.

On or about 21 December 2022, Foreman Fabricators Inc. located at 4605 McRee Avenue in Saint Louis, Missouri had not ensured that listed or labeled equipment was used or installed in accordance with any instructions included in the listing or labeling. A relocatable power tap (RPT) was connected to an extension cord which was connected in series to another extension cord to power a grinder.

29 CFR  1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  On or about 21 December 2022, Foreman Fabricators Inc. located at 4605 McRee Avenue in Saint Louis, Missouri had not ensured cords had proper strain relief.  A Valley Electric Corp. Buffer, S/N 69740, did not have proper strain relief on the power cord.
