29 CFR  1926.451(g)(4)(xv):Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches (0.8 M) above the work platform or as a toprail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart:  a) On or about December 22, 2022, on the west side of the building, where employees were applying stucco from the second tier of a scaffold without midrails.

29 CFR  1926.451(e)(8):Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:  a) On or about December 22, 2022 on the west side of the building, employees were exposed to fall hazards, when crossing a gap greater than 14 inches between scaffolds to enter and exit the work area.

29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) On or about December 22, 2022, on the south side of a building, where and employee was applying plaster to the molding of the building from a JLG 450AJ SLL Aerial Lift, without being tied off.
