29 CFR  1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  Location: 16 Hilltop Drive, Burlington, MA  On or about 12/22/22, employees were not adequately protected against falls of 10 feet or greater above a lower level in the following instances.  Instance a: Employees working from the lower roof (front porch roof) were exposed to fall hazards.  Instance b: Employees working on a second floor were exposed to fall hazards from an open and unguarded window opening.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  Location: 16 Hilltop Drive, Burlington, MA  On or about 12/22/22, employees were observed utilizing a portable extension ladder that did not have 3-feet of side rail extension above the upper landing surface it was being used to gain access to.
