29 CFR  1926.102(a)(2):The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  Location: ground  On or about 12/21/22, employees were exposed to eye injuries from flying objects when cutting boards with a circular saw without eye protection.

29 CFR  1926.501(a)(2): Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity:  Location: site On or about 12/21/2022, the employer did not ensure the second-floor deck had the strength and structural integrity while plumbing up which caused a partial collapse of the structure exposing employees to struck-by/crushing hazards.

29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Location: second floor deck On or about 12/21/22, employees were not protected from falling when working on a flat deck 16 feet above a lower level.
