29 CFR 1926.651(c)(2): A stairway, ladder, ramp, or other safe means of egress was not located in the trench excavation that was 4 feet or more in-depth for employees to egress when required. Employees were exposed to engulfment hazards.  On or about January 3, 2023, two employees were in a trench at an area that was over five feet deep laying a water line without having a means of egress, exposing the employees to a cave-in hazard.

29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).   On or about January 3, 2023, employees were laying pipe in a trench that was over five feet deep without the use of a protective system, exposing the employees to a cave-in hazard.
