On top of a structure at 400 Matilda Lane, Jacksonville, FL: On or about January 4, 2023, the employer exposed an employee to a fall hazard, in that the employer did not protect the employee from a 20-foot exterior fall hazard by use of a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On top of a structure at 400 Matilda Lane, Jacksonville, FL: On or about January 4, 2023, the employer exposed employees to a fall hazard, in that a Little Giant ladder was positioned in the interior of a structure for employees to access the 2nd level landing of a structure. The ladder did not extend 3 feet above the landing, exposing employees to a fall hazard.
