29 CFR 1910.29(b)(1):The top edge height of top rails, or equivalent guardrail system members, were not 42 inches, plus or minus 3 inches, above the walking-working surface: A) 110 Newfield Ave. Edison, NJ 08837 - Warehouse The employer did not provide adequate fall protection for the employees doing inventory from a platform attached to a powered industrial truck. The top chain across the front of the platform had sufficient sag as to not meet the nominal 42 inches height requirement; thus exposing the employee to a fall hazard of 12 feet. Condition(s) were observed on or about 1/04/2023.

A) 110 Newfield Ave, Edison, NJ 08837 - Warehouse The employer did not provide adequate fall protection for the employees doing inventory from a platform attached to a powered industrial truck. The lower chain across the front of the platform had sufficient sag as to not meet the nominal 21 inches of height midway between the top guardrail and working surface; thus exposing the employee to a fall hazard of 12 feet while working from the elevated platform. Condition(s) were observed on or about 1/04/2023.

Both forks of the powered industrial truck were not properly placed within the designed lifting pockets under the base of the platform to ensure the platform was secure and balanced.
