29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)  On or about January 5, 2023, where employee(s) were exposed to fall hazards while working more than 5-feet above the ground level below without the use of a guardrail system or personal fall arrest system.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  a)	On or about January 5, 2023, where employees were exposed to fall hazards while using a ladder that did not extend at least three (3) feet above the landing and was not equipped with a grab rail.
