29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: a) 31556 Oak Orchard Drive in Millsboro, DE 19966: On or about January 4, 2023, employees used ladders that did not extend at least 3 feet above the upper landing to access the upper levels of a multi story building, exposing them to fall hazards approximately 24 feet above the ground.
