On or about January 6, 2023, at the above addressed jobsite, an employee performing residential roofing operations, nine (9') feet one (1") inches above the ground, was not protected from falls, thereby exposing employees to fall hazards. b. On or about January 6, 2023, at the above addressed jobsite, an employee performing residential roofing operations, seventeen (17') feet five (5") inches above the ground, was not protected from falls, thereby exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) a. On or about January 6, 2023, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
