29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a)  Jobsite located at 249 Powell Rd, Springfield, PA - On or about January 9, 2023, the employer did not ensure that employees were protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure. Employees were conducting roofing operations without the use of fall protection, thereby exposing employees to falls of approximately 20 feet.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite located at 249 Powell Rd, Springfield, PA- On or about January 9, 2023, employer did not train an employee conducting roofing activities exposed to fall hazards in the recognition and mitigation of fall hazards, thus exposing employees to falls of approximately 20 feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladders were used to gain access.  a) Jobsite located at 249 Powell Rd, Springfield, PA - On or about January 9, 2023, the employer did not ensure the side rails of ladders used to access the roof extended at least 3 feet above the upper landing surface, thereby exposing employees to fall hazards.
