Inside LDS Temple Form Structure: Employees used a walk behind floor grinder machine to level out the concrete floor while an employee of the general contractor assisted by wetting the area with a  water hose to control dust emissions. The engineering controls listed in Table 1 for a walk behind floor grinder were not properly implemented. Employees were exposed to respiratory hazards.
