29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Workers were exposed to fall hazards at a worksite at 1630 S Killingsworth Ave, Bolivar MO 65613. This was last demonstrated on or about September 9, 2022, when workers were using a JGL aerial boom lift. Workers using the lift were exposed to fall hazards greater than 40 feet from the eve of the roof to the ground below. Workers were wearing no safety harnesses or lanyards in the lift while it was in use.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Workers were exposed to fall hazards at a worksite at 1630 S Killingsworth Ave, Bolivar MO 65613. This was last demonstrated on or about September 9, 2022, when workers were engaged in roofing activities. Workers removing roofing materials were exposed to fall hazards greater than 40 feet from the eve of the roof to the ground below. No guard rails, safety net system(s), or personal fall arrest system(s) were utilized to prevent falls.
