29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:   Site- On or about December 22, 2022, employees installing exterior sheathing were not protected from a fall hazard while working from a job-made wooden box being supported by a JCB rough terrain forklift that was not designed for hoisting personnel.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  Site- On or about December 22, 2022, employees were not protected from a fall of approximately 15 feet while working from a job-made wooden box with an open front that was supported with a rough terrain forklift.
