29 CFR  1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  a) Xtreme Motorcycle Outlet located at 198 Penn Street, Brilliant, OH 43913:  On an preceding January 10, 2023, the employer did not ensure that employees regularly operating forklifts throughout the facility completed training and evaluation to ensure competency.  Two employees were tasked with unloading delivery trucks and moving products throughout the facility up to 4 hours per day using two different forklifts.  The employees were not trained and evalualuated prior to being tasked with operation thereby exposing them and others throughout the warehouse to struck-by hazards.
