Scranton St. in Ravenna OH, that was 4 feet (1.22,) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for the employer working within the trench excavation.

29 CFR  1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use:  On or about January 10, 2023, the employer did not ensure the ladder used at the job site located at the intersection of Main St. & N. Scranton St. in Ravenna OH, was inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use. The ladder used at the job site by the employee to enter into the trench had structural defects such as damages to the side rail.

On or about January 10, 2023, the employer did not ensure an employee working inside a trench ranging in depths of approximately 58 inches to 6 feet 3 inches located at the intersection of Main St. & N. Scranton St. in Ravenna OH, was protected from cave-ins by an adequately designed protection system.
