29 CFR 1910.147(c)(1): The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperable. a) On or about 01/10/2023 throughout the facility, employer failed to establish an energy control program that consists of energy control procedures, employee training and periodic inspections to ensure employees use LOTO procedures to control energy while doing maintenance activities such as but not limited to: changing dies on Minster P2-30, replacing grinder wheels on Okamoto grinder, changing blade on bandsaw, changing belts on sanders, and changing grinding segments on Blanchard machine. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 01/10/2023, in the Press Room, when employees perform maintenance and servicing activities, such as changing the dies, lace the machine, refitting air cylinders on Minster P2-30-30, energy control procedures were not being developed and utilized, exposing employees to unexpected start of the machine. b) On or about 01/10/2023, in the Grinder Room, when employees perform maintenance and servicing activities, such as changing the grinder wheel on Okamoto Grind-X, energy control procedures were not being utilized, exposing employees to unexpected start of the machine. c) On or about 01/10/2023, in the Blanchard area, when employees perform maintenance and servicing activities, such as changing segments on the Blanchard Machine serial number 11260, energy control procedures were not being developed and utilized, exposing employees to unexpected start of the machine. ABATEMENT DOCUMENTATION REQUIRED
