The employees were performing work near the unprotected edges of the home. Thereby exposing employees to falls of approximately 13 foot 4 inches to the ground below. The employees were also working at or near the unprotected stairwell opening to the basement below without any fall protection system in place. Thereby exposed the employees to falls of approximately 20 feet to the basement floor.  b. Located at Lot 97 at 54 Chamber Bay Road, Commercial Point Ohio 43116: On or about January 10, 2023, employees were performing work on the ground level of the home and had no fall protection system in place around the stairwell opening to the basement below. Thereby exposing the employees to falls of approximately 9 foot 11 inches to the basement floor below.

29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a. Located at Lot 97 at 54 Chamber Bay Road, Commercial Point Ohio 43116:  On or about January 10, 2023, an extension ladder was used to access the second story of a two-story home. The ladder did not extend 3 feet above the access point and did not have any secured support at the top of the ladder, thereby exposing the employees to a fall hazard of approximately 13 feet 4 inches to the ground below.
