29 CFR  1926.651(b)(3):When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means:  On or about January 11, 2023, the employer did not determine by a safe and acceptable means the exact location of the gas line. The gas line was struck by a 2019 Bobcat, Mini Excavator (Model: E32i, Serial # *B3Y112907*) being used to bore/dig out the excavation. This condition exposed employees to struck-by and/or explosion hazards.
