On or about January 16, 2023, employees were expanding an excavation to install a drainage pipe in a trench 4 feet or more in-depth that did not have a safe means of egress.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).   On or about January 16, 2023, employees were prepping and excavating for the installation of a drainage pipe in a trench greater than five feet in depth that was not protected from cave-ins by an adequate protective system.
