29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a)601-624 (Building 6), Felix Way, Westampton, NJ 08060: A worker was observed working underneath a pump jack scaffold without head protection. The worker was not wearing a hard hat to protect them from falling objects when another worker was working above. Observed on or about 01/17/23. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(4)(i):Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) Building 6 Felix Way, Westampton, NJ: The employer did not ensure the safety net(s) and end caps were beign utilized and/or installed properly on the Pump Jack Scaffold, exposing employees to falls from a height of approximately 30 feet high. Observed on or about 01/17/23. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

The ladder was straddling the crest of a 10:12 pitched entryway roof with the bottom rung of the ladder sitting on the crest. Observed on or about January 17th, 2023. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
