29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:   Instances including, but not limited to, the following:  a) Job site, 220 Baywood Avenue, Mount Lebanon, PA: On or about January 17, 2022, during residential construction activities, the employer did not ensure that scaffold systems had proper access in that one section of scaffolding had a pick set up on the outside of the ladder, and at least one employee climbed over it to get on the scaffold.  b) Job site, 220 Baywood Avenue, Mount Lebanon, PA: On or about January 17, 2022, during residential construction activities, the employer did not ensure that scaffold systems had proper access in that one section of scaffolding had a ladder being used for access/egress that was only extended approximately 1 1/2 feet above the scaffold platform.

29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   a) Job site, 220 Baywood Avenue, Mount Lebanon, PA: On or about January 17, 2022, during residential construction activities, the employer did not ensure that scaffold systems were properly inspected for safety hazards. Deficiencies noted during the inspection included, but were not limited to, scaffolding was too close to a live electrical service drop, there was not a safe way to get on/off scaffolding and no fall protection was in use when working at heights of over 10 feet.
