29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about January 18, 2023 the employer Toledo Sign Company, at a jobsite located at 27322 Carronade Perrysburg OH 43551, did not ensure that an employee was protected from falls while operating from a truck mounted scaffold work platform. One employee was exposed to falls of approximately 10. 8 feet to the ground below.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about January 18, 2023 the employer Toledo Sign Company, at jobsite located at 27322 Carronade Perrysburg OH 43551, did not ensure employees were protected from falls while performing commercial sign installation activities atop a low sloped commercial awning. One employee was exposed to 10 foot and 8 tenths or greater fall to the ground below.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about January 18, 2023 the employer Toledo Sign Company Inc., at jobsite located at 27322 Carronade Perrysburg OH 43551, did not have a fall protection training program in place to enable each employee to recognize the hazards of falling, and procedures to minimize these hazards. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.
